SLSQ Blue Card Application Form - Please use this form if you do not hold a Blue Card and need to apply for one. Please fill out Part B of the form only making sure your signature is within the black box. After you have completed this bring the form into the Noosa Heads SLSC Administration office to have your form processed. You will need to bring suitable identification as set out in Part C of the form.
Volunteer Blue Card Application Form
SLSQ Blue Card Renewal Form - Please use this form if you hold a Blue Card but it is about to expire or has already expired. Please fill out Part B of the form only making sure your signature is within the black box. After you have completed this bring the form into the Noosa Heads SLSC Administration office to have your form processed. You will need to bring suitable identification as set out in Part C of the form.
Volunteer Blue Card Renewal Form
SLSQ Blue Card Authoristation Form - Please use this form if you applied for and were issued with a Blue Card from another organisation (e.g. Football Club, Scouts). This form authorises us to check the validity of your Blue Card as per the organisations requirements. Please bring your Blue Card Authorisation along with your current Blue Card to the Noosa Heads SLSC Administration office for collection and processing.
